Home and community habilitation and companion services for people with intellectual disabilities ages 21 years of age and older.
Individuals receive the support they need to live in their own homes or apartments in the community or with their natural family. Support is provided on an individual basis.
Services could include: bathing, morning or evening routine assistance to promote independence, assistance with learning daily living skills and oversight of medical needs and/or opportunities for community integration through community social interaction.
